To participate in CYBHI and access reimbursements, your district will need to complete several essential documents. Here's a breakdown of each one, along with some key considerations:
- Data Use Agreement (DUA): This agreement outlines how student data will be shared and protected, ensuring compliance with privacy regulations like HIPAA and FERPA.
- Tad Tip: The DUA often requires board approval, so it's essential to begin this process early and ensure you have all the necessary documentation for your board meeting.
- Provider Participation Agreement (PPA): This agreement establishes the terms and conditions for providers offering services under CYBHI, including qualifications, responsibilities, and reimbursement rates.
- Tad Tip: Ensure that all providers, both internal and external, fully understand the terms of the PPA. Misunderstandings can lead to compliance issues and delays in reimbursement.
- Secure File Transfer Protocol (SFTP) Form: This form sets up the secure electronic transfer of data between your district and the relevant agencies, ensuring that sensitive information is transmitted safely.
- Tad Tip: Work closely with your IT department to complete this form accurately. Incorrectly configured SFTP can lead to data breaches or rejected submissions.
- SPI Roster: The Student Provider Identifier (SPI) Roster is a list of all students receiving services under CYBHI, along with their assigned provider identifiers.

- Provider Connect Form: This form is used to register and manage provider information within the CYBHI system.
